The objective of this research was to investigate the diagnostic accuracy of post-mortem ultrasound in antral follicle count (AFC) determination and compare it with visual AFC in grazing crossbred Holstein cows, at high altitude in Ecuador. Pre-mortem blood from 80 cows was collected, and AFC and ovarian characteristics were analysed post-mortem by ultrasound and visual techniques. AFC counts were stratified as high, medium or low by terciles. Mean AMH concentration in pre-mortem blood was 280.1 ± 15.53 pg/mL. The AFC obtained by visual inspection (26.9 ± 9.49 follicles) was 23.8% higher than by ultrasound (20.5 ± 7.53 follicles) in all ovaries. Body condition score, age and weight of the cattle did not interact with the count technique. In the low AFC group, visual inspection and ultrasound provided similar AFC results. However, in the Medium- and High-AFC groups, AFC by ultrasound was 14.9% lower than AFC by visual inspection. We confirm that ultrasound can be used with great accuracy for AFC >3 mm (close to the resolution limit) in grazing crossbred Holstein cows at high altitude.

Introducción: La miocardiopatía hipertrófica (MCH) constituye una enfermedad compleja y variable en cuanto a morfología, fisiopatología, pronóstico y sobrevida. Se caracteriza por una hipertrofia del ventrículo izquierdo, aunque en ocasiones puede ser biventricular o ventricular derecha aislada. En la edad pediátrica predominan las causas no sarcoméricas. La ecocardiografía es el método diagnóstico más utilizado para caracterizar la morfología y repercusión hemodinámica, sin embargo, la resonancia magnética cardíaca (RMC) es de elección por aportar mayores ventajas morfológicas y funcionales. Las pruebas genéticas son de gran importancia en la determinación del diagnóstico etiológico, manejo terapéutico y pronóstico. El tratamiento tiene como objetivo principal mejorar la capacidad funcional y aliviar los síntomas. Los betabloqueantes no vasodilatadores son considerados de primera líneay en algunos casos se puede recurrir a terapias avanzadas como disopiramida o terapia de reducción septal (TRS), uso del cardiodesfibrilador implantable (CDI), y en ocasiones se debe considerar trasplante cardiaco en pacientes con síntomas refractarios al tratamiento médico y en MCH del tipo obstructivo. Materiales y métodos: Estudio observacional, descriptivo, corte transversal. Resultados: Se estudiaron 12 pacientes, desde enero de 2020 a diciembre de 2022, con edades comprendidas desde 5 meses hasta 16 años, diagnosticados con MCH de presentación clínica variable y de distintas etiologías. Conclusión: La MCH de causas no sarcoméricas como los síndromes malformativos y errores innatos del metabolismo son las que tienen peor pronóstico y sobrevida. La finalidad de detectar precozmente la MCH es el inicio temprano de la terapia específica, para así retrasar el compromiso cardiovascular y mejorar la sobrevida.


Introduction: Hypertrophic cardiomyopathy (HCM) is a complex and variable disease in terms of morphology, pathophysiology, prognosis and survival. It is characterized by left ventricular hypertrophy, although it can sometimes be biventricular or isolated right ventricular. In the pediatric population, non-sarcomeric causes predominate. Echocardiography is the most widely used diagnostic method to characterize morphology and hemodynamic repercussions; however, cardiac magnetic resonance imaging (CMR) is preferred because it provides greater morphological and functional advantages. Genetic tests are of great importance in determining the etiological diagnosis, therapeutic management and prognosis. The main objective of the treatment is to improve functional capacity and alleviate symptoms. Non-vasodilator beta-blockers are considered first-line treatment and in some cases advanced therapies such as disopyramide or septal reduction therapy (SRT), use of implantable cardioverter-defibrillator (ICD) can be used; heart transplantation should be considered in patients with symptoms refractory to medical treatment and in HCM of the obstructive type. Materials and methods: This was an observational, descriptive, cross-sectional study. Results: 12 patients were studied, from January 2020 to December 2022, with ages ranging from 5 months to 16 years, diagnosed with HCM of variable clinical presentation and of different etiologies. Conclusion: HCM from non-sarcomeric causes such as malformation syndromes and inborn errors of metabolism have the worst prognosis and survival. The purpose of early detection of HCM is the early initiation of specific therapy, in order to delay cardiovascular compromise and improve survival.

El presente trabajo muestra cómo se adaptó a enseñanza online un módulo de formación laboral para jóvenes con discapacidad intelectual que participan en un programa universitario de formación para el empleo, esta adaptación emana de la crisis sanitaria producida por la COVID-19. El artículo busca conocer cómo este grupo de jóvenes evalúa la experiencia de formación online y comparar la satisfacción percibida en la formación presencial y online. Metodología: La investigación obedece a un estudio de tipo descriptivo transversal, a través de dos cuestionarios ad-hoc cumplimentados por 16 estudiantes. Resultados: Muestran que la formación a distancia tiene más ventajas que desventajas; los estudiantes autoinforman que afianzaron elementos relativos a búsqueda activa de empleo, habilidades de autogestión y manejo autónomo de programas digitales; además se manifiesta que no existen diferencias significativas respecto a la satisfacción entre modalidad virtual y presencial, tanto en satisfacción con las actividades como en la relación con los docentes. Conclusión: Los participantes están abiertos a la formación online y valoran de forma positiva el apoyo directo. Se vislumbra que Internet otorga oportunidades de desarrollo autónomo al colectivo, la formación mixta les permite poder generar otros usos a Internet, permitiéndoles enriquecer sus habilidades digitales y laborales. This work shows a job-training module was adapted to online learning for young people with intellectual disabilities participating in a university job skills pro-gramme, this adaptation emanating from the health crisis produced by COVID-19. The article aims to find out how this group of young people evaluate the experience of online training and to compare the perceived satisfaction in face-to-face and online training. Methodology: The research follows a cross-sectional descriptive study, through two ad-hoc questionnaires completed by 16 students. Results: The results show that distance learning has more advantages than disadvantages; the students self-report that they have strengthened elements related to active job search, self-management skills and autono-mous management of digital programmes. Furthermore, there are no significant differ-ences in terms of satisfaction between the virtual and face-to-face modes, both in terms of satisfaction with the activities and the relationship with the teachers. Conclusion: The participants are open to online training and positively value direct support. It is seen that the Internet provides the group with opportunities for autonomous development; blended learning allows them to generate other uses for the Internet, enabling them to enrich their digital and work skills. Abstract Cardiovascular disorders represent the leading cause of death in dialysis patients. Alterations of bone and mineral metabolism (BMM) and vascular calcifications play a fundamental role in it. The objective of this study was to evaluate the predictive role on cardiovascular mortality of the measurement of biomarkers of BMM and vascular calcifications. A prospective cohort study was performed. All prevalent patients on chronic dialysis in September 2009 at our institution, who completed the total of the complementary stud ies, were studied. BMM biomarkers were measured (FGF 23, fetuin A, PTH, calcium and phosphorus) and the vascular calcifications were evaluated using the Kauppila and Adragao scores. Follow-up was carried out until 1/1/2019, death or transplant. Of the 30 patients included, 7 (23.3%) died due to cardiovascular causes. The follow-up time was 44.1 ± 30.4 (range = 1.4-112) months. The Adragao score was the only predictive variable of long-term cardiovascular mortality (area under the curve = 0.82; 95% CI 0.64-0.94; p < 0.001). The best cut-off point was 5 (sensitivity = 85.7%; specificity = 78.3%). It was also an independent risk factor for cardiovascular mortality adjusted for age, diabetes mellitus, coronary heart disease, aortic calcifications, time spent on dialysis and follow-up time (adjusted OR = 1.77; 95% CI = 1.06-2.96; p = 0.028). The vascular calcifications quantified from the Adragao score were the only independent predictor of long-term cardiovascular mortality. This score represents a simple, useful and superior tool to the biomarkers of BMM.


Resumen Los trastornos cardiovasculares representan la primera causa de muerte en los pacientes en diálisis. Las alteraciones del metabolismo óseo y mineral (MOM) y las calcificaciones vasculares juegan un papel fundamental en la misma. El objetivo de este estudio fue evaluar el rol predictor sobre la mortalidad car diovascular de la medición de los biomarcadores del MOM y las calcificaciones vasculares. Se realizó un estudio de cohorte prospectivo. Se estudiaron todos los pacientes prevalentes en diálisis crónica en septiembre del 2009 en nuestra institución que completaron el total de los estudios complementarios. Se midieron biomarcadores del MOM (FGF 23, fetuína A, PTH, calcio y fósforo) y se evaluaron las calcificaciones vasculares mediante los scores de Kauppila y de Adragao. Se realizó un seguimiento hasta el 1/1/2019, la muerte o el trasplante. De los 30 pacientes incluidos, 7 (23.3%) fallecieron por causa cardiovascular. El tiempo de seguimiento fue de 44.1 ± 30.4 (rango = 1.4-112) meses. El score de Adragao fue la única variable predictiva de muerte cardiovascular a largo plazo (área bajo la curva = 0.82; IC95% = 0.64-0.94; p<0.001). El mejor punto de corte fue de 5 (sensibili dad = 85.7%; especificidad = 78.3%). RESUMEN Introducción: la evaluación y el manejo del suicidio, en el contacto inicial con un profesional de la salud en el servicio de urgencias, es una importante intervención preventiva y curativa. Se ha demostrado que el servicio de urgencias tiene el potencial de identificar casos de suicidio en varios contextos y realizar intervenciones que salven vidas. Objetivo: determinar los conocimientos y prácticas de médicos de urgencias de un Hospital Universitario sobre la atención de pacientes suicidas. Metodología: estudio observacional exploratorio, piloto, descriptivo, de corte transversal, con muestreo no probabilístico. Se encuestó a médicos que trabajan en el servicio de urgencias del Hospital de Clínicas de la Universidad Nacional de Asunción, utilizando para ello una versión adaptada del instrumento elaborado por Betz y colaboradores. Resultados: 67 médicos participaron de la investigación, 52,20 % eran del sexo femenino, con edades comprendidas entre los 24 y los 43 años (media de 30,5±6,3 años), 33 eran médicos internos, 18 eran médicos residentes, y 16, médicos especialistas. La mayoría estaba de acuerdo/muy de acuerdo con que los suicidios pueden prevenirse. En todos los niveles de formación, la mayoría de los participantes no estaba seguro de poseer los conocimientos necesarios para reconocer pacientes con ideas suicidas, sobre su habilidad para evaluar la gravedad del riesgo de suicidio, ofrecer consejería o ayudarle a crear un plan de seguridad. Conclusiones: la mayoría de los médicos de urgencias no se encuentra seguro de sus conocimientos para la atención y la evaluación de pacientes suicidas. Se debe establecer un programa de capacitación continuo, que incluya habilidades para la mejora de los conocimientos y las prácticas relacionadas con la atención de pacientes con riesgo suicida. Este programa podría formar parte de un módulo de educación médica continua, que los médicos de urgencias deberían cursar y aprobar.


ABSTRACT Introduction: The assessment and management of suicide, in the initial contact with a health professional at the emergency department, is an important preventive and curative intervention. The emergency department has been shown to have the potential to identify suicide cases in various settings and to deliver life-saving interventions. Objective: To determine the knowledge and practices of emergency physicians of a University Hospital regarding the care of suicidal patients. Methodology: This was an observational, exploratory, pilot, descriptive, cross-sectional study, with non-probabilistic sampling. Physicians working in the emergency department of the Hospital de Clínicas of the National University of Asunción were surveyed, using an adapted version of the instrument developed by Betz et al. Results: Sixty seven physicians participated in the research, 52.20% was female, aged between 24 and 43 years (mean: 30.5 ± 6.3 years), 33 were interns, 18 were resident physicians, and 16 were specialists. Most participants agreed/strongly agreed that suicides can be prevented. At all levels of training, most participants were unsure whether they had the knowledge to recognize suicidal patients, about their ability to assess the severity of suicide risk, offer counseling, or help patients to create a safety plan. Conclusions: The majority of emergency physicians are not sure of their knowledge for the care and evaluation of suicidal patients. A continuous training program should be established, including skills for the improvement of knowledge and practices related to the care of patients at risk for suicide. This program could be part of a continuing medical education module, which emergency physicians should complete and pass.

BACKGROUND: There are few data in Argentina on the prevalence and management of bone and mineral metabolism (BMM) in patients with chronic kidney disease (CKD). OBJECTIVES AND METHODS: A survey was carried out in dialysis units in 2010 to measure the prevalence of and types of treatments for BMM disorders in Argentina. The data obtained was then compared to the published results from other large population studies. We recorded characteristics of dialysis centres and participating patients, the frequency of measurements and individual results for BMM biochemical markers, as well as the type of management used to control hyperphosphataemia and secondary hyperparathyroidism. RESULTS: 1210 patients from 25 dialysis centres in Argentina participated in the study (representing 4.7% of the country's prevalent dialysis population in 2010). The mean patient age was 55.3±17.6 years, 60.8% were male, 3.3% were on peritoneal dialysis and 29.1% suffered diabetes. In all centres, phosphataemia and calcaemia were measured on a monthly basis, 60% of centres measured intact parathyroid hormone (iPTH) every 6 months, 36% every 3 to 4 months, and 4% annually. As recommended by K/DOQI, 51.6% of patients had adequate levels of calcium (8.4-9.5 mg/dl), 51.6% had adequate phosphorus (3.5-5.5 mg/dl) and 21.1% displayed acceptable iPTH levels (150-300 pg/ml). 24% had iPTH <150 pg/ml and 54.5% >300 pg/ml. iPTH ≥600 pg/ml was present in 28.3%, and 13.3% had values ≥1000 pg/ml. These figures differed from those published by the DOPPS II study, in which 51.1% of patients had iPTH <150 pg/ml, and only 26.7% had iPTH ≥300 pg/ml. Calcium-based phosphate binders were used in 83.6% of the patients, 5.6% used sevelamer and 4.0% used aluminium-containing compounds. To achieve control of hyperparathyroidism, oral or intravenous calcitriol was predominantly used (50.5%) with a small percentage of patients receiving paricalcitol or doxercalciferol. CONCLUSIONS: The present study shows a high prevalence of secondary hyperparathyroidism, which differs from that published by other large population studies. There was a high proportion of patients with BMM markers outside the ranges suggested by K/DOQI. Mainly phosphate binders based on calcium and calcitriol continue to be used for the management of hyperphosphatemia and hyperparathyroidism respectively.

ANTECEDENTES: Existen pocos datos acerca de la prevalencia de los trastornos del metabolismo óseo y mineral (MOM) y su forma de manejo en pacientes con enfermedad renal crónica en Argentina. Objetivos y métodos: Mediante una encuesta realizada en 2010 en centros de diálisis, determinamos la prevalencia y las formas de tratamiento de los trastornos del MOM en Argentina y los comparamos con los datos publicados por otros grandes estudios poblacionales. Consignamos las características de los centros de diálisis y de los pacientes participantes, la frecuencia de las determinaciones y los resultados individuales de los marcadores bioquímicos del MOM y el tipo de manejo utilizado para controlar la hiperfosfatemia y el hiperparatiroidismo secundario. RESULTADOS: Participaron 1210 pacientes de 25 centros de diálisis de 10 provincias de Argentina (lo cual representaba el 4,7 % de la población prevalente en diálisis del país en 2010). La población estudiada tenía una edad media de 55,3 ± 17,6 años, 60,8 % eran varones, el 3,3 % en diálisis peritoneal y el 29,1 % eran diabéticos. El 100 % de los centros determinaban calcemia y fosfatemia mensualmente, el 60 % hormona paratiroidea intacta (PTHi) semestralmente, el 36 % cada 3 o 4 meses y el 4 % de forma anual. Según las recomendaciones de K/DOQI, el 51,6 % de los pacientes tenían niveles adecuados de calcio (8,4-9,5 mg/dl), el 51,6 % de fósforo (3,5-5,5 mg/dl) y el 21,1 % de PTHi (150 a 300 pg/ml). El 24,4 % tenían PTHi < 150 pg/ml y el 54,5 % > 300 pg/ml, con un 28,3 % con valores de PTHi > 600 pg/ml y un 13,3 % > 1000 pg/ml. Estos datos diferían de los publicados por el estudio DOPPS II, donde el 51,1 % de los pacientes presentaban PTHi < 150 pg/ml, y solo un 26,7 % PTHi > 300 pg/ml. El 83,6 % utilizaban un captor del fosfato basado en calcio, el 5,6 % sevelamer y el 4,0 % compuestos con aluminio. Para el control del hiperparatiroidismo se utilizaba predominantemente calcitriol oral o endovenoso (50,5 %), con un pequeño porcentaje de pacientes recibiendo paricalcitol o doxercalciferol. CONCLUSIONES: El presente estudio muestra una elevada prevalencia de hiperparatiroidismo secundario, lo cual difiere de lo publicado por otros grandes estudios poblacionales. Existe una elevada proporción de pacientes con marcadores del MOM por fuera de los niveles sugeridos por K/DOQI. Para el control de la hiperfosfatemia y el hiperparatiroidismo, se continúan utilizando mayormente captores del fosfato basados en calcio y calcitriol, respectivamente

In all centres, phosphataemia and calcaemia were measured on a monthly basis, 60% of centres measured intact parathyroid hormone (iPTH) every 6 months, 36% every 3 to 4 months, and 4% annually. As recommended by K/DOQI, 51.6% of patients had adequate levels of calcium (8.4-9.5mg/dl), 51.6% had adequate phosphorus (3.5-5.5mg/dl) and 21.1% displayed acceptable iPTH levels (150-300pg/ml). 24% had iPTH <150pg/ml and 54.5% >300pg/ml. iPTH >600pg/ml was present in 28.3%, and 13.3% had values >1000pg/ml. These figures differed from those published by the DOPPS II study, in which 51.1% of patients had iPTH <150pg/ml, and only 26.7% had iPTH >300pg/ml. Calcium-based phosphate binders were used in 83.6% of the patients, 5.6% used sevelamer and 4.0% used aluminium-containing compounds. To achieve control of hyperparathyroidism, oral or intravenous calcitriol was predominantly used (50.5%) with a small percentage of patients receiving paricalcitol or doxercalciferol. CONCLUSIONS: The present study shows a high prevalence of secondary hyperparathyroidism, which differs from that published by other large population studies. There was a high proportion of patients with BMM markers outside the ranges suggested by K/DOQI. Mainly phosphate binders based on calcium and calcitriol continue to be used for the management of hyperphosphatemia and hyperparathyroidism respectively

The water channel aquaporin-1 (AQP1) is the molecular counterpart of the ultrasmall pore that mediates free water transport during peritoneal dialysis (PD). Proof-of-principle studies performed in rats have shown that treatment with corticosteroids upregulates the expression of AQP1 in the peritoneal capillaries, causing a significant increase in free water transport. Whether such a beneficial effect could be observed in end-stage renal disease patients treated by PD remains unknown. Peritoneal transport parameters were evaluated in three patients on PD, shortly before and after living-donor renal transplantation and treatment with high-dose methylprednisolone (1.0-1.2 g/m(2)). As compared with pre-transplantation values, the post-transplantation test revealed an ∼2-fold increase in the sodium sieving and ultrasmall pore ultrafiltration volume, suggesting an effect on AQP1 water channels. In contrast, there was no change in the parameters of small solute transport. The direct involvement of AQP1 in these changes is suggested by the expression of glucocorticoid receptors in the human peritoneum and the presence of conserved glucocorticoid response elements in the promoter of the human AQP1 gene.

We evaluated the efficacy of percutaneous ethanol injection therapy (PEIT) as a therapeutic option for recurrence of secondary hyperparathyroidism after subtotal parathyroidectomy in ESRD patients. Six patients underwent PEIT. A mean of 1.3 ± 0.8 ethanol injections was performed. Nodular volume was 1.5 ± 1.7 cm(3), and 2.8 ± 2.8 cm(3) of ethanol was injected per patient. After ethanol injection PTH decreased significantly (1897 ± 754 to 549 ± 863 pg/mL (P < .01)). There was also a reduction in serum calcium, phosphorus and calcium-phosphorus product. A positive and significant correlation was found between nodular volume with ethanol injected and time from parathyroidectomy. Only one patient required hospitalization due to severe hypocalcaemia. In other two cases, local discomfort and temporary mild dysphonia were registered. PEIT is an effective treatment to control recurrences of secondary hyperparathyroidism postsubtotal parathyroidectomy.

Serum creatinine is an insensitive marker to identify early changes in glomerular filtration rate (GFR), for this reason alternative methods to estimate renal function result of great clinical importance. Forty-one patients were studied using creatinine clearance modified with cimetidina (Clcrc) as surrogate of GFR, cystatin C-based equations (i.e. Larsson and Hoek formulas), Cockroft-Gault and MDRD abbreviated equations. In the whole group, as well as in those patients with serum creatinine < or =1.2 mg/dl--but reduced renal function: Clcrc 62.01 +/- 17.33 ml/ min/1.73 m(2)-, Larsson and Hoek equations showed higher correlations and lower bias than creatinine-based formulas. Abbreviated MDRD equation showed good performance just in those patients with evident alteration of renal function (serum creatinine > 1.2 mg/dl). We concluded that in patients with different stages of renal function, cystatin C-based equations detect reduction of renal function earlier than the serum creatinine-based formulas.

La creatinina sérica es un marcador poco sensible para identificar reducciones leves del índice de filtración glomerular (IFG); por ello resulta de gran importancia clínica disponer de métodos alternativos para estimar la función renal. Con este objetivo estudiamos la función renal de 41 pacientes -grupo completo y divididos según la creatinina sérica (menor o igual 1.2 mg/dl o mayores)- usando el clearance de creatinina modificado con cimetidina (Clcrc) como aproximación al IFG, las ecuaciones de Larsson y Hoek que incluyen el uso de cistatina C sérica y las tradicionales fórmulas de Cockroft-Gault y MDRD abreviada. En el grupo completo de pacientes y especialmente en aquellos con creatinina sérica menor o igual 1.2 mg/dl - con reducción de la función renal: Clcrc: 62.01 mas o menos 17.33 ml/min/1.73 m2-, las ecuaciones de Larsson y Hoek mostraron mejores correlaciones y menores diferencias promedio respecto a las fórmulas basadas en la creatinina sérica. La ecuación MDRD abreviada mostró buen rendimiento sólo en el grupo con evidente alteración de la función renal (creatinina sérica > 1.2 mg/dl). Concluimos que en pacientes con diferentes estadios de función renal, las fórmulas que emplean la cistatina C sérica detectan la reducción del IFG más precozmente respecto a aquellas basadas en la creatinina sérica.
